A model of care for the treatment of knee pain in older adults in primary care has recently been published in Rheumatology (link to free access below). The model was developed after a review of published recommendations for knee osteoarthritis and a consensus exercise held at the 2004 PCR Society Annual Meeting. The review identified 27 interventions and these have been arranged (by consensus) into a four stepped model, with step 1 to be offered to all sufferers and interventions in further steps to be considered if there is ongoing pain and disability The review included papers up to May 2004 and some of the recommendations may need to be reviewed in light of further research evidence (e.g. chondroitin) but broadly the model presents a practical approach to managing this often ignored problem presenting in primary care.
